Community Residential Service

Our licensed residential group homes provide a safe, supportive, and nurturing environment for individuals with developmental disabilities. Services are available 24 hours a day, 7 days a week, and include assistance with activities of daily living, personal care, laundry, housekeeping, and household responsibilities, based on each individual’s assessed needs and personal goals. Supports are delivered in a manner that promotes independence, dignity, choice, and quality of life.

Supported Living

Supported Living Services provide individualized supports to individuals with developmental disabilities who live in their own homes or with family. Services are designed to promote independence, self-direction, and community integration while assisting with activities of daily living, household management, budgeting, health and safety, and community participation based on each individual’s needs and goals.

Respite Care Services

Respite Services provide short-term, planned, or emergency supports to individuals with developmental disabilities and their caregivers. Services may be provided in one of Primus Health Inc.’s licensed residential homes or in the individual’s own home, based on need and availability. Supports are delivered in a safe and supportive environment and are tailored to meet each individual’s needs while ensuring continuity of care, safety, and overall well-being.

Personal Supports

Personal Support Services assist individuals with activities of daily living and personal care needs, including hygiene, meal preparation, mobility, medication reminders, and health and safety monitoring. Supports are delivered in a respectful, person-centered manner that promotes independence, dignity, and individual choice.

Community Development Services

Community Development Services support individuals in building social connections, developing life skills, and increasing participation in community activities. Services focus on fostering independence, relationship-building, and meaningful engagement within the community while respecting individual interests and goals.

Employment Services

Employment Services support individuals with developmental disabilities in exploring, obtaining, and maintaining competitive or supported employment. Services may include career exploration, job readiness training, job coaching, workplace supports, and collaboration with employers to promote long-term success and inclusion in the workforce.

Nursing Support

Nursing Support Services provide skilled nursing oversight and health-related supports to ensure the safety and well-being of individuals served. Services may include health assessments, medication management oversight, care coordination, health education, and collaboration with interdisciplinary teams and medical providers in accordance with individual service plans and applicable regulations.

Day Habilitation

Day Services offer structured and meaningful daytime supports that promote skill development, socialization, and community engagement. Individuals are supported in developing life skills, pursuing personal interests, and participating in community-based activities that enhance independence, choice, and overall quality of life.
